Achievements: Please describe below the member's achievements that are to be recognized and how the member has fulfilled the criteria for the chosen award nomination, why it is important and how the nominee's actions have contributed to the profession or practice of local government. Nominations should be between 1 and 3 pages. Alternatively, you may upload a document below. Note, for Long Service award nominations only employment history is required as supporting information.

Your nomination will be submitted to the Awards Selection Committee for consideration. Those nominees selected for an award will be contacted and invited to the members recognition luncheon at the LGMA annual conference to receive their award.
